Your First 2 Visits

1st Visit Upon
entering my office you will be greeted cordially by Carol, Yonia,
Janice, Nina, Francesca or Diana and asked to
complete a few health information forms. These forms give me vital
information regarding your past and present health concerns so that
I can give you the best care possible. It only takes a few minutes
to complete. For those patients who have suffered auto injuries please
be sure to bring the accident report with you (if it has been
completed), and any auto insurance information you have. ( Claim
number, name and address of your company, and telephone number).
There will be an additional form to complete in the case of an auto
accident. But, again, it is easy to fill out. Any other health
insurance information is normally given to the office during this
visit so it can be determined what type of coverage you
have.
After speaking with me and giving me a
detailed health history, I will ask you specific questions regarding
your current health issue. Samples of questions to think about would
be&what type of pain do you have, sharp, achy, muscular; where
is the pain located; are there any movements or activities that
increase the pain, or for that matter, make the pain go away; what
type of treatment have you used so far for this condition, etc.
Please be prepared to answer these questions as they help me
determine the type of examination needed in your
case.
Typically, a physical examination is
performed. Height, weight, blood pressure, pulse will be taken,
along with other standard testing. The area in question will be
examined with certain orthopedic/neurological tests performed. In
most cases when pain is present xrays are taken in my office. If you
have reasonably current xrays of the injured area, please bring them
with you. In some cases these xrays can be used rather than taking
new ones. Also, my office has computerized Range of Motion equipment
in which I can perfoem studies on the spot.
Once the examination and xrays are
completed, unless you are in severe pain, this will complete the
first visit. It is my feeling that it is important for me to study
the findings of the xrays and examination prior to beginning
treatment. For those patients who are in significant pain, I will
attempt to help you with pain relief that day. A convenient
appointment will be given the next office day to go over the test
results. Your spouse is invited and encouraged to attend the second
visit.
2nd
Visit The purpose of the second
visit is to explain, in detail, the findings of all tests performed
on the first visit.
Any
insurance information we have received will be reviewed with you so
that you will know what your insurance company says your financial
obligation will be for the treatment you are receiving. You will
also have ample opportunity to ask questions to your
satisfaction.
Once
all of these topics are explained, I have reserved time to treat you
on the second visit so that we can start making progress
immediately.
It is
my feeling that while it is my job to report to you the type of
treatment I feel is the best for your condition, it is ultimately up
to you to determine to what degree you want treatment and how fast
you want results. I feel confident that I can customize your care to
give you the best opportunity to achieve the results you desire. If,
in your particular case, I have reservations about accepting your
case or determine that chiropractic cannot help you, I will do my
best to communicate this to you so that we can, together, determine
what is your best alternative. |
